Traditional Plan -
Comprehensive Master Medical (CMM)
The Blue Cross and Blue Shield of Michigan Health
Care Program (BCBSM) covers all eligible hospital, medical
and surgical expenses subject to an annual deductible and co-payment.
After satisfying the deductible, the insured pays 20% of additional
charges up to a maximum of $1,000 per family per calendar year.
There is a separate prescription drug benefit. Co-payments
for mental/nervous conditions and private duty nurses are 50%/50%,
which does not apply to the $1,000 annual maximum.
Plans are available to groups of
2 or more.
Eligibility:
All active Association members in good standing and their full-time
employees are eligible to apply. A spouse and unmarried dependents
(to age 25) may be added to the member or employee's policy.
Michigan residency is required.
Features:
• No medical underwriting required
• Rates based on one person, two persons, family, and family continuation
• Eight (8) geographical rating areas in Michigan
• Individual contract deductible options: $1,000
• Group contract deductible options: $250 or $500

2. Community Blue PPO
Combines the benefits of traditional care with
the wellness and prevention features of managed care. You may
elect to use an in-network physician with low out-of-pocket
expenses and preventative care. Or you may choose an out-of-network
physician with higher out-of-pocket expenses.
Eligibility:
All active Association members in good standing and their full-time
employees are eligible to apply. A spouse and unmarried dependents
(to age 25) may be added to the member or employee's policy.
Michigan residency is required.
Features:
• Flexibility - in and out of Network care covered
• Basic benefits - covered with no deductible and no % co-pay
• Individual contract deductible option: $500
• Group contract deductible options: $0, $100, $250, $500, $1,000
• Prescription drug co-pay options:

Individuals: $10/60
- Groups: $10/20, 10/40, 15/30, 15/50, 15/30/60 or
50%/50%
• Lifetime maximum benefit of $5,000,000 per member
3. Blue Care Network (HMO)
This plan emphasizes preventative care. Provides
services for hospital care, physician services (including
routine care), diagnostic testing and prescription drugs
with minimal co-payments.
Eligibility:
All active Association members in good standing and their
full-time employees are eligible to apply. A spouse
and unmarried
dependents
(to age 25) may be added to the member or employee's
policy. Michigan residency is required.
Features:
• Managed health care
• Choice of nominal co-pays per service
• Basic benefits covered with no deductible or co-pay
• Preventative Services
• No medical underwriting required if transferring from another BCBSM group
