Death and Dismemberment
This program is endorsed by your association.
Accidental Death and Dismemberment (AD & D) will pay a benefit for
accidental loss of life or bodily injury occurring within 180 days of
the accident. Bodily injury involves various amounts for single and double
dismemberment and loss of sight or speech or hearing.

Eligibility:
Members and their employees under age 70 are eligible. A member’s
spouse and any unmarried, dependent children to age 19 (to age 23 if
a full time student in an accredited college or university) are eligible
to participate in a family plan with the member.
The Family Plan covers your spouse for 40% of your benefit and eligible
dependent children for 5% each of the member’s benefit.
