available for eligible non-profit and for-profit entities.

For underwriters to consider offering terms, the following
information may be necessary:
1. A completed D&O application
2. Copies of the by-laws and Articles of Incorporation
3. Latest Interim Balance Sheet and Income Statement
4. List of the directors and officers
5. If publicly traded, include the latest 10-K, 10-Q, and Proxy Statement
Completing and submitting an application does not obligate the applicant to purchase
any insurance that may be offered.
